Job Description:
Spectrum Healthcare Resources is seeking a dedicated Registered Nurse (RN) Case Manager to join our team at Hickman Air Force Base in Pearl Harbor, Hawaii. As an RN Case Manager, you will play a crucial role in providing comprehensive care management services to our military personnel and their families. RN Position Details: Ind.Nurse
- Full-Time, Monday-Friday, 9 hour shifts
- No Weekend OR On-Call responsibilities!
- Any State License Accepted
- All paid Federal Holidays
- Opportunity to provide care for Active Duty Military and their dependents
- Excellent compensation ranging from $87,000 to $89,200
Qualifications:

Degree/Education: Shall be a graduate from an associate (ADN) or baccalaureate degree (BSN) program in nursing accredited by a national nursing accrediting agency recognized by the US Department of Education

Certifications: In addition to Basic Life Support, candidates must possess one of the following certifications:
- Certified Registered Nurse Case Manager by Commission for Case Management (CCM) or American Nurses Credentialing Center (ANCC) -OR- certification eligible with 3 years of experience for ADN or 2 years of experience for BSN in full time clinical case management with children, families, seniors, or groups.
- In addition, Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS) and Neonatal Resuscitation Course (NRC) certifications shall be as required by AFI 44-102 or equivalent

Experience: A minimum of 3 years for ADN or 2 year for BSN full-time experience in clinical case management working with adults, children, families, seniors and groups. Nurses with familiarity with the military healthcare system and telehealth experience is desirable. Nurses with work experience with Tri-care, and Military Health System Genesis systems preferred.

Licensure/Registration: Current, full, active, and unrestricted license
Key Responsibilities:

Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care.

Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring.

Identify opportunities for CM and integrate local CM processes into practice.

Develop and implement local strategies using inpatient, outpatient, onsite, and telephonic CM.

Create and implement tools to support case management, such as patient identification and assessment tools, clinical practice guidelines, algorithms, CM software, and databases for community resources.

Integrate CM with utilization management (UM) and collaborate with social work case management for holistic care solutions.
- Collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care
- Support for the patient and family/caregivers to ensure identified education and appropriate, timely care is received
